This educational conference is designed to provide knowledge and insight to professionals working in the health care field regarding dementia topics such as how to use therapeutic services with patients and clients, understanding their legal needs, managing client fears about dementia and information about the new Dementia Plus program.

Patricia Stern, MSW, LCSW, MBA, PMP, CSM is a Clinical Social Worker and President of Patient Centered Care Partners with a broad background in health care venues, wellness, benefits, behavioral health and health care delivery systems. Ms. Stern has extensive social work experience as a consultant to nursing homes and medical home care agencies. She is a private practitioner in a psychotherapy practice and created the first Department of Social Services in a telehealth care disease management organization. Her professional titles include Marketing Manager Home Services, Advocate Health Care/Condell Medical Center; Director Advocacy Services, Trustmark Companies/Health Contact Partners; Program Director of Compassionate Care Hospice and Clinical Program Manager, BlueCross BlueShield of Illinois, Montana, New Mexico, Oklahoma and Texas.
