The scenarios address identification of methamphetamine labs, opiate addiction, suicidal persons, and various other potential risks to staff and children in the field, in a real life setting. As staff progress through the scenario, role players react to the staff and staff complete scenarios as they would in the field, contacting supervisors, emergency services or law enforcement. 3:30 PM - 4:30 PM
Capgras in the Courtroom: Delusional Misidentification Triggered by Forensic Testimony in a Patient with Chronic Schizophrenia
A 68-year-old man with chronic schizophrenia developed a transient Capgras-like delusion after observing his psychiatrist testify in favor of civil commitment. This case illustrates neurobiological and psychodynamic mechanisms underlying delusional misidentification and underscores the forensic challenges legal proceedings pose to maintaining therapeutic alliances in severely psychotic individuals.
Emotional stress during legal proceedingsโ€”especially when involving trusted cliniciansโ€”can precipitate transient psychotic phenomena such as Capgras-like delusions in individuals with schizophrenia. Clinicians should anticipate and monitor for such reactions during high-stakes events like commitment hearings.
This case demonstrates that strong therapeutic rapport can persist even in the presence of misidentification delusions. It also underscores the complexity of transference in psychosis and highlights opportunities to maintain engagement despite distorted perceptions and how role conflict may harm the therapeutic relationship.
