Event Details

For various reasons, Case Managers may struggle to close individuals on their caseload, despite being the optimal time. In this Case Manager-led training discussion, participants will learn about positive ways to work toward successful case closures. Discussion topics covered include timely and effective re-engagement processes, common closure strategies and pitfalls, and the use of resources to aid in the closure process. The SAM western region workgroup will provide participants with suggestions, showcase tactics through role-playing scenarios, and review everyday closure situations to help build a case manager's skillset.

Melissa’s current work includes supporting staff and individuals we serve. Melissa is a team player and seeks new opportunities to learn and grow.

    My name is Jamie Hulbert and I have been with SAM for almost 7 years. I started in Family Services and moved to the mental health field in 2018. Since being in the mental health field, I have become part of a great team. I became a Master Blended Case Manager in 2021. I have always had a passion for helping others and this job allows me to do just that. I have been able to feed this passion by participating in numerous work groups with Sam’s Western Region, I have been fortunate enough to attend National Association of Case Management conferences, and have had many training opportunities. The passion for helping others doesn’t stop at work, I have a very active home life as well. I married my high school sweetheart in 2005 and together adopted our beautiful daughter who is now 12. When we aren’t busy with sporting events, we are spending time with family, kayaking, side-by-side riding, or planning events at church. I have two main jobs at church, secretary and live streaming our services for others to enjoy.

Nicole is passionate about nurturing all dimensions of wellness, empowering individuals to foster change and growth, and finding time to laugh and connect with others. Nicole is also a certified Yoga, Pound Pro and Generation Pound Pro, and Group Aerobics Instructor.
