Active Member This is a 12-month membership - Membership expires Dec 31st This membership is for individuals only An applicant for membership must be a resident of the Western Section which consists of the states of Alaska, Arizona, California, Hawaii, Idaho, Montana, Nevada, Oregon, Utah, Washington and Wyoming; the provinces of Alberta, British Columbia and Saskatchewan in the Dominion of Canada, and the Pacific Island possessions of the United States of America. Requirements (1) Certification by the American Board of Urology, Inc. or the Royal College of Surgeons of Canada. (2) Limitation of the applicant's practice entirely to the specialty of urology for at least one (1) year within the geographical area of the Western Section. (3) Certification in writing by the applicant that he/she has read, understands and agrees to abide by the By-Laws of the Western Section. (4) Possession of an unlimited license to practice medicine and surgery in the State, Province or Country of residence, and possession of an M.D. or D.O. degree. (5) In special instances, the Board of Directors may waive a qualification or requirement and recommend for active membership a Urologist whose position and achievement, in its opinion, warrants such action. (6) The Board of Directors may waive all or part of the requirements for Active Membership for individuals who have been certified or recertified by the American Board of Urology within the past 18 months. (7) If an Active Member fails to become recertified as required by the American Board of Urology (or other certifying board) he/she shall be transferred to Associate Member status. (8) If an Active Member becomes decertified by the American Board of Urology (or other certifying board) he/she shall be automatically dropped for non-compliance with AUA and Section bylaws, pursuant to expulsion and reinstatement policies | $0 |
