The research results serve as a basis for the general surgery fraternity to negotiate fair reimbursement with regulatory bodies and medical aid companies. It is essential that surgeons, as with any other profession, understand and accurately determine their value in the South African surgical workforce in order to prepare their profession for the imminent changes in healthcare payment structures. It is of fundamental importance that developments in the healthcare payment system in the country are based on an accurate assessment of the economic value and level of complexity of the work being done by general surgeons.

Government Employees Medical Scheme – Ophthalmology Claims Experience The Government Employees Medical Scheme (GEMS) is the largest closed medical scheme in South Africa. GEMS provides cover to over 2.0 million beneficiaries. Via an expert multidisciplinary team, which includes clinicians, actuaries and other professionals, GEMS closely monitors its claims experience. This monitoring allows for strategic interventions aimed at improving access to high quality care whilst simultaneously containing the cost of care. In the context of ophthalmology, significant variations in the cost of care are observed across regions. This is even after accounting and adjusting for variations in the clinical and demographic profile of beneficiaries. Variations extend to both the level of activity and the costs associated therewith. This begs the question as to why such variations exists and whether these variations suggest that there is an opportunity to better standardise the care which is delivered to patients. Standardisation can help to contain the cost of care and to further extend access to care.

Brenda's training includes Basic International ICD-10 training (National Centre for Classification in Technology, Brisbane, Australia) and Advanced International ICD-10 training with Eve Jones from the UK. In 2013 Brenda was appointed by the Minister of Health to be part of the Ministerial ICD-10 Task Team and to continue as chairperson of the Training Sub-Committee. Read Bio Brian Warren Brian Warren’s basic medical training was at the University of Cape Town, where he graduated in 1974. After a few years of compulsory and voluntary life and professional experience, he went on to specialise in Surgery at Stellenbosch University, obtaining the qualifications MMed(Chir) and Fellowships of the College of Surgeons of South Africa and the Royal College of Surgeons of Edinburgh. Apart from some brief forays into limited private practice, his specialist career was spent fulltime on the joint Provincial/University staff of the Department (later Division) of Surgery at the Tygerberg Campus of Stellenbosch University, where he was appointed Head in January 2002. With restructuring of the Faculty in 2006, he was also prevailed upon to head the newly-created Department of Surgical Sciences. While he trained and was a young specialist in the “general” era, his main clinical interest was always surgery of the gallbladder and biliary tract, which combined well with the advent of laparo-endoscopic surgery in the early 1990’s. Other abiding interests include undergraduate/postgraduate teaching and bioethics/medical law. These pursuits, particularly the latter, have kept him busy since mandatory retirement from fulltime practice early in 2016.
